Patient Information
Most insurance plans are accepted if you are covered by a dental insurance plan. A PPO insurance plan gives you the freedom to choose a dentist in or out of network. We will gladly file your insurance forms for you with a request to have the benefit assigned to the office. Bucks Dental Associates will never try to provide you with services that you do not need. Our best interest is our patients and we are here to provide you with excellent care and manage your dental necessities.
In-office Insurance Plan- An Invitation for Membership
Bucks Dental Associates at Chalfont PA are pleased to invite you to become a member of our personalized dental benefits plan. For the cost of $450.00 per year, you will receive the following:
- 1st visit- $450.00 includes full mount X-rays, cleaning, exam, oral cancel screening, personal oral hygiene instruction and treatment planning when necessary and 1 fluoride treatment.
- 2nd visit-$0.00 recall exam and cleaning (must be made on the same day of the 1st visit)
- Discounted rate on most dental work(not including Nitrous, Arestin, Varnish, Whitening services)
Notes
We urge you to keep any HMO or managed care medical insurance you now have. You may need it for medical/ hospital expenses, pharmacy expenses, etc.
Your membership is only valid at the Chalfont office. You may want to keep any other dental insurance you have in case you move out of the surrounding area or if you require a dental specialist.
Our office does accept most major dental PPO and Discount insurances; however, our customized membership plan will provide you with the quality dentistry that you deserve.
This invitation is made available to patients that do not carry dental insurance or do not wish to utilize it but are interested in a discounted membership program. (This membership may not be used as a secondary insurance.)

Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
